When torsion of the cyst (tumor) legs is primarily disturbed its nutrition and develop symptoms characteristic of torsion.
The severity of the clinic is not alwaysis determined by the degree of torsion. Sometimes, even with a relatively small twisting of the leg (90-120 °), severe symptoms of the disease appear, while at a greater degree of torsion (sometimes even up to 360 °) the symptoms of the disease may be absent or remain unexpressed.
In the case of acute crooked cysts(swelling) of the ovary suddenly there are severe pains in the abdomen (especially in the lower parts) of a paroxysmal nature, radiating to the legs and lower back, as well as nausea, vomiting of a reflex character, stool and gas retention. Sharp pain syndrome may be accompanied by symptoms of shock: pallor of the skin and mucous membranes, cooling of the limbs, frequent pulse, a sharp decrease in blood pressure, etc. The abdomen is swollen, palpation marks the stiffness of the muscles of the anterior abdominal wall and symptoms of irritation of the peritoneum (picture of an acute abdomen). The temperature is normal or subfebrile, there is leukocytosis in the blood, the ESR increases.
If the torsion of the ovarian cyst (tumor) torsion occurs gradually, the symptoms are less pronounced, the pain is localized in the area of the tumor location; the symptoms either abate or reappear.
With gynecological research in the field ofappendages of the uterus is defined by a tumor-like formation of an oval, tight or densely elastic consistency, with a smooth surface, limited mobility, sharply painful upon palpation and displacement. An important sign of twisting the cyst (tumor) is the increase in its size due to edema and venous plethora. This sign is of great diagnostic value in those cases when the patient is under systematic medical supervision. The ovarian cyst is usually located posteriorly and somewhat laterally from the uterus (usually there are only dermoid cysts in front of the uterus). The uterus and appendages on the other hand are usually unchanged. Because of the strain and soreness of the anterior abdominal wall, it is often necessary to resort to the study of a patient under anesthesia.
Diagnosis with acute twisting of the leg usually does not cause difficulties. In the anamnesis there is an indication of the presence of a cyst or a tumor.
The diagnosis is facilitated by the detection of a cyst (tumor), which is sharply painful when displaced and palpated. The picture of internal bleeding with this disease is absent.
